Inequality in physical activity: evidence from Global Matrix 5.0 in the Basque Country

Congratulations to the Basque Country Report Card team on their recent publication titled “Inequality in physical activity: evidence from Global Matrix 5.0 in the Basque Country,” published in Retos! The paper analyzed inequalities in physical activity of children and youth in Euskadi. The abstract and citation are below.

Abstract

Introduction. Regular physical activity during childhood and adolescence has been associated with numerous physical, psychological, and social benefits. However, levels of inactivity remained high and varied according to sex, age, and socioeconomic status.

Objective. The objective of this study was to analyze the inequalities in physical activity of the child and adolescent population of Euskadi, within the framework of the Global Matrix 5.0 initiative, identifying differences by sex, age and socioeconomic level, as well as their evolution with respect to the 2018–2021 cycle.

Methodology. The research was based on a descriptive and comparative analysis conducted by a panel of six experts from academic and governmental institutions. Twenty-three sources of information covering the period 2022–2025 were compiled using a multilevel strategy that integrated scientific literature, institutional reports, and accelerometry studies. Eleven indicators were evaluated using the standardized criteria of the Active Healthy Kids Global Alliance and scored on a scale of A+ to F according to the degree of compliance.

Results. The results showed improvements in overall physical activity (B+/B), physical fitness (C+), and public policies (B), while sleep and sedentary behaviors received the lowest ratings (F). Girls showed a slight advantage in overall physical activity and active transportation, but adolescence reflected the greatest declines, especially in socioeconomically disadvantaged contexts.

Conclusions. These findings are consistent with previous European studies showing a progressive decline in physical activity with age and a sustained increase in sedentary behavior. It is concluded that the Basque Country has made progress in public policies and the promotion of physical activity, although challenges related to sleep, sedentary behavior, and social equity persist.
